Test Center Feedback Form

ACT welcomes feedback about your test day experience. If you experienced a problem on test day or have a concern about your test center, please describe it in detail. We will investigate the situation and respond to you, normally by mail. Therefore, an accurate mailing address is required. You must complete this form no later than 2 weeks after you test.

What is my ACT ID and why is it important?

If you register for an ACT National or International Test Date, we will automatically assign you an ACT ID whether or not you provided your Social Security number (SSN) when you registered. If you test through any other testing program (e.g., State, Special, Arranged, DANTES, or Residual), we assign an ACT ID only if you did not provide your SSN. The ACT ID is an eight-digit number beginning with a dash that is located at the top of your Student Report and also appears on the score reports we send to the high school and colleges you requested.

The ACT ID helps us identify your record if you did not provide your SSN and allows you to access and order ACT services without having to provide your SSN. (For example, if you wish to send your scores, correct information on your score reports, or create a student Web account.)

If you test through State, Special, Arranged, or Dantes Testing, you can view your scores and score report online after you receive your score report in the mail. All you need to do is create or log in to your ACT Web account. You must provide the ACT ID printed on your score report to access your scores if you did not provide your SSN when you tested.
